COE 360 is designed to deliver access to the best care by bringing together the most highly rated healthcare facilities, surgeons, and physicians in the country and leading-edge virtual providers that can meet a person or their loved ones where they are and make sure they get the help they need.

Designed alongside clinical partners, this program provides access to treatment tailored to the individual. Care includes virtual, home-based care through category pioneer Lionrock and in-person treatment through the world-renowned Hazelden Betty Ford.* This approach allows the individual the flexibility to maintain their family and work obligations while also focusing on getting better. The Contigo Health® Centers of Excellence 360 Substance Use Disorder Program offers all of this and a team that surrounds the person throughout their treatment journey on their terms with a program that aligns with their lifestyle.

For members who are looking for a surgical solution for long-term weight loss, Contigo Health’s Weight Loss Surgery Program can be a great option. Contigo Health’s network of bariatric surgeons are thoroughly vetted and reviewed, are known for outstanding safety ratings, and have a patient-first approach to weight loss surgery. Members will qualify for the program if they meet one of the following criteria:

• Member has a BMI of 35 or more regardless of presence, absence, or severity of co-morbidities.
• Member has metabolic disease and a BMI of 30 or more.
• Member is of Asian descent and has a BMI of 27.5 or more.
