Walmart is committed to providing quality care for its health plan members (eligible Associates and dependents). That is why Walmart is offering a special program for hip replacement surgeries, knee replacement surgeries, spine surgeries, and weight loss surgical procedures. Please note: Coverage for spine surgeries and weight loss surgeries is available only under this COE program; in certain circumstances a network exception may apply for spine surgery. Hip and knee replacements not performed through the COE program will have a reduced benefit, unless a network exception applies.

The program covers services for members at a COE facility that has a track record of quality results. If the member qualifies, the Walmart health plan pays up to 100% of eligible costs* for most conditions, and even covers travel and lodging for the member and a caregiver when using the COE program. Members must be enrolled in the Premier, Contribution, or Saver plans or a Local plan to be eligible for these benefits.

*Deductibles may apply for members with HSA Saver Plans, and coverage may vary by program type and clinical needs. The weight loss surgery program is subject to deductible and coinsurance amounts. See the 2024 Associate Benefits Book for more information about the benefits and eligibility.

For members who are looking for a surgical solution for long-term weight loss, the Weight Loss Surgery COE Program can be a great option. The program’s network of bariatric surgeons are thoroughly vetted and reviewed, are known for outstanding safety ratings, and have a patient-first approach to weight loss surgery. Members will qualify for the program if they meet the following criteria:

1. Had a primary care provider (PCP) visit in the 30 days prior to COE program intake

 
AND one of the following:

2. Has a body mass index (BMI) of 40 or higher OR
3. Has a body mass index (BMI) of 35 or higher and at least one or more obesity-related comorbidities, such as type 2 diabetes, hypertension, sleep apnea or other respiratory disorders, nonalcoholic fatty liver disease, osteoarthritis, lipid abnormalities, gastrointestinal disorders, or heart disease
